Introduction: Chronic rhinosinusitis with nasal polyps (CRSwNP) is associated with depression and anxiety. In this study, we explore whether the initiation of dupilumab is associated with a lower incidence of depressive episodes and acute care utilizations among CRSwNP patients with underlying depression.

Methods: A retrospective cohort study from 2015-2024 using the TriNetX Research Network identified individuals with CRSwNP using ICD-10-CM codes J33, J32, excluding cystic fibrosis and congenital respiratory malformations. Dupilumab-treated (DT) patients were compared with non-dupilumab (ND) patients, with the index date defined as dupilumab initiation or a matched encounter. Both cohorts had pre-index depressive disorders. Propensity score matching (1:1) was performed on age, sex, race, and ethnicity. Outcomes assessed 6–12 months post-index included depressive episodes, anxiety disorders, emergency department (ED) visits, and inpatient encounters.

Results: Before matching, 1975 DT patients and 29,293 ND patients were identified. After matching, there were 1975 patients in each cohort with no significant differences in demographics. The ND cohort had a higher risk of depressive episodes compared with DT patients (429 [21.7%] vs 335 [17.0%]; RR 1.28, 95% CI 1.13–1.46; p<.01). No significant difference in the incidence of other anxiety disorders (407 [20.6%] vs 402 [20.4%]; p=.84) was observed. ND patients had higher rates of ED visits (217 [11.0%] vs 147 [7.4%]; RR 1.48, 95% CI 1.21–1.80; p<.01) and inpatient encounters (186 [9.4%] vs 129 [6.5%]; RR 1.44, 95% CI 1.16–1.79; p<.01).

Conclusions: Dupilumab’s impact in CRSwNP may extend beyond sinonasal symptom control to broader patient-centered outcomes. Understanding these outcomes may help guide holistic counseling and follow-up strategies for patients with CRSwNP.
