(647) Salivary Gland Malignancy Outcomes at a Safety-Net Hospital for a Predominantly Minority, Disadvantaged Patient Cohort

Introduction: This study describes clinical outcomes and treatment delivery for Salivary Gland Malignancies (SGM) in a predominantly minority, underserved population treated at a safety-net hospital.

Methods: Retrospective analysis was performed for patients with SGM between 2017 and 2025. The Area Deprivation Index (ADI) was used to measure neighborhood socioeconomic disadvantage, with higher scores indicating greater disadvantage. Survival outcomes were evaluated using Cox regression and log rank testing.

Results: 47 patients were included (57.4% male; 59.6% Hispanic, 30% black). Many resided in socioeconomically disadvantaged neighborhoods (mean ADI 68.1). Disease burden was high: 66.0% T3-T4 tumors and 31.9% nodal positivity. High grade tumors (38.3%) were significantly associated with greater neighborhood disadvantage (median ADI 77 vs 64; p=0.03). 91.5% of all patients (M0 and M1) underwent surgical resection, and positive margins occurred in 30.2% of cases. 68% of all patients underwent adjuvant radiation. For the non-metastatic cohort (n=38), 5-year overall survival (OS) was 71.4% and disease-free survival (DFS) was 66.0%. 5-year locoregional control was 94.1%. Recurrences were predominantly distant (7.9%). Male sex was associated with more advanced stage at diagnosis (88% vs. 55%) and significantly worse OS (p=0.011) & DFS (p=0.034). Multivariate analysis showed nonsignificant trends for advanced T-stage and node positivity toward decreased DFS.

Conclusions: In this predominantly minority, socioeconomically disadvantaged cohort of patients treated at a county, safety-net hospital, the majority of patients presented with locally advanced disease. High-grade tumors were associated with increased ADI. Nearly all patients received standard of care surgery with or without adjuvant radiation. Further studies should evaluate the link between male sex and worse outcomes.
